If you would like the Clinic to bill your insurance, please download the Insurance Benefit Check Form and complete it with the help of a representative from your insurance company. The form provides a guide for questions to ask the representative in order to obtain a comprehensive understanding of your insurance coverage for mental health and occupational and speech therapy. It will also help you identify any steps that need to be taken prior to getting coverage, such as insurance authorization or physician’s referral.

It may be listed on your insurance card or it may not. Do not hesitate to call us and ask if we are a preferred provider with your insurance network. Also, our contracts change regularly.
